Prevalence and Outcome of Obstructed Labour at a Tertiary Institution

Obstructed labour is a common preventable causes of both maternal and perinatal morbidity mortality in developing countries affecting 3-6% labouring women globally accounts for an estimated 8% deaths Sub-Saharan Africa South Asia. Objective: To determine the prevalence outcome obstructed Gynaecology Obstetrics Department at tertiary hospital Northern Uganda. Method: This was retrospective chart review pregnant admitted delivered by caesarean section from 1st January 2016 to 31st December 2017 Lira Regional Referral Hospital, institution Lango Sub region. Data analysed using Statistical Package Social Sciences version 16.0. Results: A total 808 medical charts mothers with were retrieved, evaluated included this out 12,189 deliveries during study period, giving 6.6%. Majority (77%; 622/808) diagnosis referred peripheral facilities. Over half (53.4%) age group 20 29 years. 53%, prime gravidae twice more likely undergo C/S due than multigravidas (OR 1.8; 95% CI 1.5-2.2). Only 49.2% had documented cause labour, Cephalo-pelvic disproportion being most (17.5%), malposition/mal-presentation (14.6%), macrosomia (3.6%). Partograph used only 46.6% (374) who labour. The commonest complication observed Sepsis (11%), PPH (5.2%), uterine rupture (4%) burst abdomen (3%) which led prolonged stay loss fertility some. Mode delivery reviewed section, 90.3% babies born alive, while 8.5% (69) dead, (1.2%) early neonatal, 40.2% Paediatric Neonatal Intensive Care Unit (NICU) because low APGAR score. Conclusion: among Caesarean high compared global average. referrals health centres associated life threatening complications even death. Young gravida rates. obstruction multigravida could be secondary cephalo-pelvic as majority normal range weight. Low use monitor evident obstructed.
